Objective to identify hospital discharge processes that include guidance for elderly home care and the needs that arise during the care transition as identified by nurses. Method: integrative literature review, conducted by searching the databases: Virtual Health Library, Latin American and Caribbean Literature in Health Sciences, Medline and Scielo. Results: 16 articles were selected that showed 2 thematic axes. Axis 1 - Direct Care in the hospital discharge process and Axis 2 - Care Management in the hospital discharge process. Conclusion: the studies allowed to identify that the guidelines of care of the elderly at home during the hospital discharge process are given through written records and that nurses play a decisive role in this process.
