The frontal sinus is a unique anatomical entity for its complex and variable morphology, as well as its functional relevance. Lesions arising in this space are enclosed between the two sinus walls and may potentially interfere with the sinus drainage. Likewise, they may be close to the brain, especially if located laterally. VSP in this setting enables to map the lesion location over the frontal sinus and plan a surgical guide that tailors the osteoplastic flap design immediately above the lesion, minimizing the risks of violating the intracranial space. Trauma of the frontal sinus often involves the skullbase and may be the source of several complications, if not addressed promptly. In this setting, VSP allows not just a better diagnosis, but an improved treatment plan as well, both when only the anterior table is fractures, assisting the correct reduction, and also in case of the posterior wall injury, assisting sinus ostectomy during cranialization. In extensive trauma with skullbase disruption, VSP helps design personalized implants that can either be fabricated in titanium or PEEK with additive manufacturing or be intraoperatively created using dedicated molds. This chapter provides a comprehensive overview of such technologies and illustrates their methodical application in the aforementioned scenarios.
