Artificial Intelligence (AI) offers novel opportunities for medicine and biomedical research, but the development of medical AI faces significant ethical challenges, such as bias, privacy, transparency, and fairness. Teaching medical AI ethics to AI developers is particularly challenging due to the abstract nature of ethical principles and their complex application in real-world contexts. This study explores the use of Virtual Reality (VR) as an innovative educational tool. We developed Ethixperience, a VR application designed to teach medical AI researchers key ethical principles and practices, including beneficence, justice, privacy, and transparency. The application features three interactive components: Slide Sphere (a lecture introducing ethical principles), Narrative Nexus (a 2D or 3D immersive narrative demonstrating the consequences of biased AI), and Interactiverse (a hands-on exercise addressing algorithmic bias). We conducted a preliminary evaluation of the application with 10 participants using a pre- and post-test design. Results showed significant improvements in participants’ understanding of general AI ethics, justice, and transparency, with participants reporting high levels of immersion and enjoyment. This study demonstrates VR’s potential to overcome the challenges of ethics education, providing an immersive and engaging learning experience for medical AI researchers.
