The correct management of progressive collapsing foot deformity starts with a careful clinical examination including visual gait analysis. Nonoperative management with the use of insoles and braces is recommended as initial treatment regardless of the stage of the disorder or the deformity. Surgical management should be individualised and foot flexibility, age, occupation, condition surgical decision-making. In the absence of arthritis, the combination of deformities may be corrected with the use of different osteotomies – medial displacement calcaneal, lateral lengthening, plantarflexion of the medial cuneiform – and soft tissue procedures – tendon transfers, ligament repair – with the goal of a plantigrade pain-free foot. In the presence of arthritis and/or rigid deformities, a variety of arthrodesis may be utilized to restore medial arch height. Adjunctive procedures may be necessary to obtain motion and/or stability.
