Surgical correction remains the only effective treatment to correct hallux valgus, with several procedures available depending on the severity and specific pathology of the deformity. Combining soft tissue balancing and bony procedures is crucial to achieve satisfactory long-term outcomes. Among the various procedures available, the scarf osteotomy and Lapidus procedure are two commonly utilized procedures. In cases with painful arthritis or failed previous surgeries, first metatarso-phalangeal fusion offers a safe and effective alternative. The scarf osteotomy is our primarily used method for correcting hallux valgus due to its versatility and robust fixation. This Z-shaped osteotomy allows for significant lateral translation of the distal fragment, effectively realigning the first metatarsal. Due to the possibility of proximal extension, even severe deformities can be corrected. Its design creates a self-locking mechanism that enhances stability and supports early weight-bearing. The Lapidus procedure is a commonly used technique for more severe deformities and/or for first ray instability. By addressing the deformity at the first tarso-metatarsal joint, the Lapidus procedure corrects metatarsus primus varus at its source. It can be performed in a modified form, preserving mobility between the first and the second ray, or as the original technique with additional fixation between the first and second rays to address significant first ray instabilities. Choosing and tailoring the right surgical approach—using one of the many methods available—ensures optimal long-term outcomes for hallux valgus correction.
