Patient-Centered Treatment for Co-Occurring Trauma and Minority Stress

In this section, we provide two case vignettes to demonstrate the implementation of culturally-responsive, affirming, and tailored evidence-based treatments for PTSD when treating LGBTQ+ clients who present with trauma-related mental health diagnoses and co-occurring minority stress. Generally, evidence-based treatments for PTSD conceptualize PTSD as a disorder of non-recovery, maintained via emotional, cognitive, and behavioral avoidance. As such, treatments directly address avoidance and aim to reduce symptoms in an effort to promote recovery. The first vignette depicts utilization and tailoring of Cognitive Processing Therapy (CPT) with a veteran client who is a White, bisexual, cisgender man. The second vignette depicts utilization and tailoring of Prolonged Exposure (PE) with a Black, pansexual, nonbinary person. For each vignette and related treatment approach, a brief overview of the treatment, relevant mechanisms of change, and rationale for use is discussed.
