Splenomegaly can be due to a variety of causes. The first step in evaluation is a thorough patient history to elucidate the cause. Initial workup should include a complete blood count with peripheral smear as well as an ultrasound to document splenic size. In the otherwise well patient, further workup with referral to a specialist will be the first step. If there is concern for rupture, urgent referral should be made to a facility with surgical capabilities.
