Intraoperative or secondary urethra injury has been considered an indication to abandon implantation or remove the cuff. However, re-dos especially in patients with fragile urethra can be challenging and lead to intraoperative complications. In select cases and under sterile circumstances, in situ urethroplasty can be performed and the cuff can be placed or left in place. However, the cuff should be loosely left open without close contact to the urethra. After 6 weeks cuff closing surgery can be performed easily and activation of AUS can be performed after 5-6 weeks. This technique allows preserving the AUS and simplified the future procedure as particularly in complex cases the preparation of the urethras may be even more challenging in the re-do situation.
