The axillary nerve innervates the deltoid and teres minor muscles. Injury to this nerve can have devastating impact on shoulder function and the ability to reach overhead. Surgical options to restore shoulder function may include the medial pectoral nerve-to-axillary nerve transfer. Postoperative rehabilitation is identified by three phases and is based on manual muscle testing (MMT) results of the recovering deltoid and teres minor muscles. Motor reeducation focuses on donor (pectoralis) muscle activation throughout the first two phases. Pairing the donor with the recipient muscles starts with passive motion and advances to active-assisted and ultimately active motion of the deltoid and teres minor muscles. Patient education regarding timelines for recovery is included throughout the first two phases to maintain patient motivation. Trace activation of this transfer can be seen around 6 months with functional movement starting around 9 months post-surgery. Resistance exercises are the hallmark of phase 3 and are introduced if 3+/5 MMT grade is achieved.
