This chapter focuses on comprehensive discharge planning to support the successful transition of treatment from intensive clinical settings to natural environments. The authors emphasize early and ongoing transition planning as essential to maintaining behavior-change gains. Strategies include initial goal setting, structured interviews, observations, generalization programming, and stimulus control. Environmental and systemic barriers are addressed alongside practical solutions to promote continuity of care. By aligning treatment goals with real-world expectations, clinicians can improve long-term outcomes and reduce the likelihood of readmission. The chapter concludes with recommendations for embedding transition considerations throughout the treatment process.
