How Social Determinants of Health Influence Health Promotion in Long-Term Care

The social determinants of health (SDOH) are non-medical factors which directly or indirectly influence health outcomes. The SDOH include several factors related to economic stability, healthcare access and quality, neighborhood and physical environment, social connectedness and community integration, education and health literacy, food security and nutrition, and cultural and sociodemographic context. This chapter focuses on the influence of the SDOH on health promotion in long-term care (LTC). We will discuss the conceptual underpinnings of the SDOH and how they vary across the LTC continuum, ranging from residential care to home- and community-based settings. We will summarize the impact of the SDOH on health-related outcomes across the LTC continuum. Finally, we will discuss different approaches to integrate the SDOH in health promotion activities in LTC. This chapter underscores the critical importance of the SDOH to provide equitable, efficient, culturally competent, and person-centered care for older adults in different LTC settings.
