This chapter describes the diagnostic accuracy and clinical aspects of 11C-choline and 18F-choline PET/computed tomography (CT) in patients with prostate cancer, including multiple challenging patient cases. The reviewed articles and our own wide experience underline the role of 18F-choline PET/CT in the staging of prostate cancer, particularly in assessing lymph node metastases. While offering a high specificity, its application is limited by lower sensitivity. Therefore, choline PET should be used as part of a multimodal diagnostic approach. The meta-analyses, including our own large analysis, solidify the role of 11C-choline and 18F-choline PET/CT in enhancing the diagnostic pathway for prostate cancer, particularly in cases of biochemical recurrence. By providing detailed information on metastatic locations and the likelihood of recurrence, choline PET/CT assists in optimizing treatment strategies and potentially improving patient outcomes. Nowadays, 18F-choline PET has been widely replaced with more sensitive PET tracers discussed in other chapters.
