Introduction: rTMS paired with CT has been shown to be effective in enhancing cognitive functions in patients with AD. We present a case of combinatorial therapy that resulted in a rapid and significant improvement in cognitive function. Case presentation: An 88-year-old woman presented with mild-moderate stage AD on stable doses of donepezil 5 mg per day and underwent 2-month sessions of combinatorial therapy, which paired CT intervention with rTMS. The protocol of rTMS focused on six different brain regions and networks linked to cognitive abilities significantly impacted by AD dementia: Broca’s region, both the left and right dorsolateral prefrontal cortex, and the left and right inferior parietal lobe. Each day, three selected regions were administered with a total of 1200 pulses at 10 Hz and 20 pulses per train. The intensity of the rTMS was set at 80% of the patient’s resting motor threshold. The investigation’s initial outcome assessments, as well as those recorded after 30, 60, and 90 days of intervention, including the Mini-Mental State Examination (MMSE), Word list recall, Immediate recall, Trial-making test A (TMT-A), Trial-making test B (TMT-B), Delayed recall, Delayed recognition, Digit span forward, Digit span backward, and a Clock drawing test. Results: The results showed that patients maintained remarkable improvements in any cognitive domain beyond the two-month intervention. Conclusion and Discussion: This report highlights that the cognitive performance of the patient has significantly long-lasting improved. These preliminary findings should therefore encourage additional research to determine the combination’s long-term therapeutic efficacy for cognitive enhancement in AD.
