In more than 80% of cases of MBC mastectomy is the advised procedure for operable disease because of the high tumour/breast tissue ratio in males. Sentinel node biopsy has now become the standard of care for clinically node negative MBC but it may be inappropriate to extrapolate from the results of the ACOSOG Z001 in terms of subsequent management. A study from the National Breast Cancer database showed that after propensity score matching, men with 2 positive nodes had a significantly better survival after axillary clearance compared with sentinel biopsy alone (84% versus 76%). A recent meta-analysis strongly supported the use of post-operative radiotherapy for improving OS in MBC. Various reconstructive procedures have been used to achieve skin closure in advanced breast cancer but now oncoplastic procedures are being used increasingly to achieve symmetry after breast conservation for MBC. There has been no significant change in management of DCIS in the USA between 2006 and 2017 with 25% being treated by partial mastectomy and radiotherapy. Most reports of male DCIS are of single cases and it is apparent that endocrine therapy is not being widely used in those with ER+ve cases, unlike the situation regarding adjuvant treatment of DCIS in females.
