Over three decades, transradial access (TRA) has become the vascular access of choice for percutaneous coronary interventions over femoral access, with evidence of reduced mortality and major complications. However, radial artery occlusion (RAO), the most common complication of TRA, poses a sustainability issue, with success rates decreasing with each attempt. Distal radial access (DRA) has emerged as a compelling alternative, offering lower RAO rates and fewer adverse events. However, global adoption of DRA is not well documented, suggesting the need for international collaboration to improve uptake and optimize patient care. With an apparent steeper learning curve for DRA, there is a call for continued innovation and education. This chapter examines the evolution of DRA and explores future technological advances and ongoing research of this technique in the interventional field.
