I outline the development and rise in popularity of the enhanced-view total extraperitoneal (eTEP) technique for hernia repair since 2012 during my practice in India and my experience having taught a large community of surgeons to safely adopt this technique. I also outline the anatomical and surgical nuances that make eTEP a versatile yet demanding procedure. The challenges during the learning curve are highlighted, including anatomical confusion, optical entry issues, and the need for specialized equipment in regions like India. I emphasize the importance of comprehensive training, understanding of abdominal wall anatomy, and the necessity for a multidisciplinary approach to manage complications. Personal experiences illustrate both the pitfalls and successes of implementing eTEP, with data showing promising outcomes in terms of recovery speed, complication rates, and hospital stay.
