Although described at almost the same time as other minimally invasive operations, laparoscopic inguinal herniorraphy has taken longer for widespread adoption among surgeons. Once past the learning curve, MIS inguinal hernia repairs have a very low recurrence rate and a lower risk of chronic pain than open repairs. Because it does not violate the peritoneum and thus reduces the risk of port site hernias and intra-abdominal injuries, TEP has become the technique of choice for many surgeons. Limited operative field and difficulty in defining the anatomy at the beginning of the procedure, however, are potential limitations that have resulted in some surgeons preferring the TAPP approach. In this chapter, we describe in detail the main sTEPs to overcome the difficulties of the technique and perform the repair safely, including port positioning, creation of an initial preperitoneal space, plane of dissection and dissection zones, anatomical landmarks, tips and tricks for hernia repair, management of lipoma, mesh placement and fixation. In addition, we discuss the indications, limitations/pitfalls, perioperative management, and postoperative results.
