Laparoscopic ventral hernia repair (also known as IPOM, IPUM or LVHR) refers to a minimally invasive operation based on intraperitoneal mesh placement and fixation. IPUM is usually indicated for small to medium-sized defects along the midline but can also be used for off-midline hernias such as Spigelian, flank or parastomal. The key steps for an IPUM repair are as follows (1) Careful lysis of adhesions and reduction of sac contents, (2) defect closure with suture and (3) appropriate mesh placement and fixation. High-quality evidence has demonstrated lower surgical site morbidity after IPUM when compared to open approaches. More recent data from observational studies also suggests that it may decrease hospital stay. The role of IPUM has decreased in recent years, mainly due to concerns about intraperitoneal mesh placement and the development of minimally invasive techniques that allow for extraperitoneal mesh placement. Nevertheless, IPUM remains a viable option for many hernia repairs around the world.
