Visual identification of the RLN is the gold standard during thyroid surgery. However, I-IONM can assist surgeons in finding the nerve and assure normal nerve function. A correct neuromonitoring system setup is crucial to rely on I-IONM. Preoperative and postoperative laryngoscopy should be offered to all patients. This chapter presents a step-by-step description of the standardized sequence of I-IONM. Following all these steps is advised for optimal results. The stimulation probe discharge depends on the neuromonitoring step (vagus nerve or RLN). If a typical V1-R1-R2-V2 sequence is obtained after thyroidectomy, it predicts normal nerve function and regular vocal cord movements in the postoperative period.
