Dealing with older people can be extremely demanding in remote consultation. In this chapter we will briefly point out what should be considered by the physician approaching a video or telephone assessment with the patient. To begin with, the handler must talk to the patient, relying in a minority of cases on the information derived from caregivers or other figures. Second, often symptoms can be different or milder in older people, masking the urgency of their condition. Third, often older people have visual or acoustic impairment, so the handler must be careful and try to tear down the barriers in remote consultation and keep in mind that the patient may have difficulties during the consultation. Remote consultation can anyway be useful to avoid transferring patients if not necessary, by permitting a safe and effective triage, allowing a comprehensive geriatric assessment, medication review, and deprescribing and promoting monitoring and continuity of care.
