Randomized trials demonstrate that menopausal hormone therapy (MHT) prevents type 2 diabetes (T2D). This chapter discusses epidemiological and clinical evidence for the deleterious effect of menopause in predisposing to T2D and reviews the beneficial effects of MHT on glucose homeostasis, which delays or prevent T2D and improves glycemia in women with T2D. We cover the limited evidence on the effect of MHT on type 1 diabetes (T1D). We also discuss recommendation for the use of MHT for prevention on menopausal symptoms in women with and without T2D based on age and time following menopause onset.
