Population health is an emerging area in pharmacy practice due to health-care reform. Pharmacists have a unique role in population health management for people living with diabetes and can improve the quality of care received by this population. Pharmacists, pharmacy students, technicians, and residents can work as part of the health-care team to improve diabetes care for populations. This can be achieved through data mining, patient assessment, intervention, and follow-up.
