T-Cell Receptor-Engineered T (TCR-T) Cell Therapy: Advancements, Challenges, and Future Perspectives

T-cell receptor-engineered T (TCR-T) cell therapy is a promising adoptive cell therapy (ACT) for solid tumor treatment. TCR-T-cell therapy involves technology to engineer T cells that express tumor-specific TCRs, increasing their affinity for cancer antigens, which may improve their cytotoxicity. Unlike chimeric antigen receptor T (CAR-T) cells, which target only membrane antigens, TCR-T cells exhibit a broader antigen repertoire, enabling the recognition of membrane and intracellular protein antigens presented by MHC-I and expanding the potential application of TCR-T-cell therapy. Given the importance of TCR-T-cell cancer treatment, this chapter discusses the methods and platforms used to discover cancer-specific TCRs, preclinical strategies for TCR-T-cell therapy development, and an overview of the current clinical landscape of TCR-T-cell therapy. This chapter summarizes the cancer antigen classes recognized by TCR-T cells, strategies for selecting antigens, indications, and patients for clinical studies. It also addresses challenges associated with TCR-T-cell therapy, including manufacturing bottlenecks, associated toxicity, and potential resistance mechanisms. Finally, this chapter offers perspective on the future development of improved TCR-T-cell therapy.
