Plantar fasciitis (PF) is a condition causing heel pain due to microtears in the plantar fascia, a band of connective tissue that runs from the heel to the toes. The plantar fascia has two main functions: supporting the foot arch and facilitating the windlass mechanism. It is the most common cause of heel pain in adults, affecting 10% of the US population. Conservative treatment is the first-line therapy, with 90% of patients undergoing rest, NSAIDs, stretching, plantar orthosis, corticosteroid injection, extracorporeal shock wave therapy, and ultrasound therapy. If heel pain persists after 12 months of conservative treatment, surgery may be indicated. Imaging techniques, such as radiographs, magnetic resonance imaging (MRI), and ultrasonography, are used to confirm diagnosis. Plantar fasciotomy is considered the surgical treatment of reference, with better results observed with endoscopic procedures compared to open techniques. Percutaneous techniques are generally associated with transverse fasciotomy and have the advantage of being performed under local WALANT anesthesia. Ultrasound-guided plantar fasciotomy is a newer approach that avoids damage to muscles, lateral plantar nerve, and Baxter’s nerve.
