A 57-year-old male presented with a 5-year history of a red longitudinal band and splinter hemorrhages involving the left thumbnail. Physical examination showed longitudinal erythronychia and a distal subungual hyperkeratotic papule. Excision with histopathology showed focal hyperplasia, elongated rete ridges, onycholemmal keratinization, and vascular ectasia, consistent with an onychopapilloma. Longitudinal erythronychia has a broad differential, including benign and malignant conditions. In equivocal cases, diagnosis requires excision with histopathology.
