A 55-year-old male presented with a longstanding history of pruritic rash on his back and incidentally discovered V-nicking of his nails. Histologic examination of the rash was consistent with a diagnosis of Darier disease. Darier disease is a rare genetic disorder that occurs due to mutations in the ATP2A2 gene. The disease typically presents in childhood with pruritic hyperkeratotic papules coalescing into plaques in a seborrheic distribution. Nail abnormalities (such as V-nicking, brittle nails, and candy cane nails) are common.
