Holistic Psychosocial Care Plans

This chapter introduces Holistic Psychosocial Care Plans, a structured yet adaptable approach to end-of-life and grief counseling. Grounded in the Spiral Model of Grieving and Healing and guided by the 4A4C competencies (awareness, acceptance, affirmation, actualization, compassion, connection, composure, and commitment), these care plans provide practical applications for supporting individuals through bereavement, nonfinite losses, and anticipatory grief. The three systemic care plans—Relationship Peaks and Valleys, Continuing Treasure, and Acceptance and Carrying-On—offer culturally sensitive and person-centered tools. The Relationship Peaks and Valleys model facilitates emotional and cognitive processing through visual mapping, helping individuals explore the complexities of their relationships. Continuing Treasure focuses on remembrance, forgiveness, and fostering enduring connections with the deceased, while the Acceptance and Carrying-On emphasizes addressing secondary losses and developing new roles and coping strategies. While each care plan has a distinct focus, they all adopt a holistic perspective, honoring the psychological, social, spiritual, and cultural dimensions of the individual. Aligned with the Spiral Model’s recognition of grieving and healing as a dynamic and multifaceted process, these care plans empower individuals to navigate their unique experiences of loss. This chapter equips care providers with practical tools to integrate these models into counseling and therapeutic practices, fostering meaning, healing, and resilience in those they support.
