The definition and assessment of “consciousness” is still by far one of the most challenging areas of modern contemporary neuroscience. Empirically speaking, consciousness is separated into its two components namely “wakefulness,” i.e., “arousal,” referring to the “level of consciousness.” The other component of consciousness is “awareness” referring to “the contents of consciousness.” What are we exactly quantifying by the “recommended dimensionless Bispectral Index (BIS) 40-60 level”? In fact, the widely advocated BIS range of 40–60 is a post hoc value, merely suggested by the early studies as a recommendation to prevent Accidental Awareness under General Anesthesia (AAGA) and was not based upon electroencephalographic analysis. A far better analogy to describe general anesthesia is the cardiovascular system that consists of numerous variables and multiple endpoints that are separately monitored using different parameters by skilled experienced clinicians.
