The landscape of neurocognitive rehabilitation has experienced a rapid advancement in personalization, engagement and effectiveness through the introduction of wearable devices and sensors. The real-time data provides continuous feedback that ensures interventions are tailored to the unique needs of the patient, especially in conditions such as Alzheimer’s disease, stroke, and traumatic brain injuries (TBIs). The monitor activity levels and sleep patterns that are obtained via fitness trackers to EEG headsets that can track brain activity, these tools help address cognitive, motor, and emotional challenges in innovative ways. Interventions such as gait training, neurofeedback, and stress monitoring technologies empower patients to take an active role in the recovery process and help the stakeholders monitor progress in real-time. The portability and user-friendliness of these devices make the rehabilitation process accessible and care can be extended beyond clinical settings into everyday life. Simultaneously, these devices can offer clinicians an objective and dynamic way to track progress that extends beyond the subjective evaluation obtained through traditional methods. Despite the vast positive outcomes of wearable technology, there are significant hurdles that must be overcome—privacy, cost-effectiveness, and integration into the healthcare systems. The technical limitations such as battery life, data accuracy, and frequency of replacement/modifications are other persistent challenges. The emerging innovations in flexible sensors, artificial intelligence (AI), tele-rehabilitation, and virtual reality (VR) have promising future in transforming healthcare. Applying multimodal systems that combine data from different devices offers a complete picture of recovery further helping clinicians provide informed decisions that enable positive outcomes for patients. Wearable devices and sensors bridge the gap between traditional rehabilitative methods with modern technology that fosters an era of patient-centered care. The evolution of wearable technology and sensors ensures rehabilitation is more effective and empowering for patients and accessible for the process of recovery.
