Chronic kidney disease requires complex, demanding, and prolonged treatment over time and has a high incidence rate, with 8% to 10% of the global population requiring renal replacement therapy. Previous studies and various entities recommend greater individualization, integration, and proximity of care to enhance clinical information management, healthcare quality, and the implementation of sustained, person-centered interventions. Objective: To map the scientific evidence regarding nursing strategies that facilitate case management for individuals with chronic kidney disease undergoing hemodialysis. Methodololy: A scoping review followed the Joanna Briggs Institute methodology and PRISMA-ScR as a supplementary checklist. The MEDLINE (PubMed) and CINHAL (EBSCOHost) databases were used for article searches, retrospectively until 2018. Results: A total of nine articles were included, identifying nursing strategies that facilitate case management: planning, intervention, and coordination based on a structured care model; motivational interviewing for individuals on regular hemodialysis programs; implementation of health education programs; emotional and psychological support; integration of technologies; and continuous evaluation and monitoring. Conclusions: Case management was associated with improved treatment adherence and quality of life. Future research is suggested to address the main challenges individuals face on hemodialysis and the impact of care environments on nursing case management.
