The interplay between gut microbiota, postbiotics, and personalized medicine presents a transformative approach to colorectal cancer (CRC) management. Research demonstrates that the gut microbiota, a diverse community of microorganisms, plays a significant role in regulating immune responses, maintaining intestinal integrity, and influencing CRC progression. Dysbiosis, an imbalance in microbial populations, promotes chronic inflammation, oxidative stress, and tumour development. Postbiotics, the bioactive metabolites or components derived from probiotics, offer promising therapeutic benefits. Their anti-inflammatory, antioxidant, and immunomodulatory properties reduce CRC risks while ensuring safety and stability compared to live probiotics. Personalized medicine, which tailors treatments based on genetic and microbial profiles, is enhancing CRC treatment strategies. Microbiome-targeted therapies, such as ‘fecal microbiota transplantation (FMT)’ and synthetic probiotics, have shown potential to improve patient outcomes. However, the clinical application of microbiome-based therapies and postbiotics faces challenges, including treatment variability, complex regulatory frameworks, and the need for long-term studies. Ethical concerns related to data privacy, informed consent, and equitable access to these novel therapies further complicate their integration into healthcare systems. Looking forward, emerging trends in artificial intelligence and machine learning are advancing predictive models to optimize CRC treatment responses. Advances in personalized microbiome therapeutics and the integration of genetic, microbial, and environmental data are expected to refine treatment strategies, improving precision and outcomes for CRC patients. The seamless combination of these elements marks a paradigm shift in CRC care, focusing on prevention, individualized therapies, and enhanced quality of life for patients.
