The Role of Infection and Inflammation in Prostate Cancer Development

Prostate cancer (PCa) is a prevalent disease among black men and is recognized as the second most frequently diagnosed cancer. PCa is characterized by abnormal proliferation of cells within the prostate gland, which can be further compounded by inflammation leading to various complications. Modifiable and non-modifiable factors have been associated with PCa that include infections caused by bacterial, viral, and protozoan pathogens, including sexually transmitted infections (STIs), age, family history, and ethnicity. The process of inflammation is a vital defence mechanism against invasive infections. However, persistent chronic inflammation can exacerbate existing tissue damage. Scientific databases such as PubMed, ProQuest, ScienceDirect, and Google Scholar were searched for available literature on infections, inflammation, and prostate cancer. While some of the publications were directly accessed, others were obtained through citations from other sources. Relevant keywords on infection, inflammation, and prostate cancer were used as the search criteria. Researchers have found evidence suggesting that inflammation contributes to the aetiology of PCa. Studies have indicated that men with a history of STIs caused by Neisseria gonorrhoeae, Trichomonas vaginalis, Ureaplasma urealyticum, and Treponema pallidum are more prone to PCa. Chronic inflammation resulting from infections is a major risk factor for PCa. Understanding the intricate relationship between inflammatory processes in the tumour microenvironment is crucial for preventing the progression of PCa and optimizing targeted treatments. Prostatitis has been identified as a factor that may elevate the risk of developing prostate cancer. Research conducted by Daniels et al. (Urology 66:964–970, 2005) established a correlation between a history of prostatitis and the onset of prostate cancer among 5821 men aged over 65 years. Furthermore, two extensive meta-analyses revealed a significant association between prostatitis exposure and prostate cancer. The California Men’s Health Study (CMHS) also indicated a relationship between prostatitis and prostate cancer, suggesting that men with a history of chronic prostatitis face an increased risk of prostate cancer. Additionally, a study highlighted a connection between a history of clinical prostatitis and a notable rise in the likelihood of developing prostate cancer within the general population. Moreover, men who reported a history of sexually transmitted infections exhibited a heightened risk for prostate cancer. Both acute and chronic inflammation may arise from pathogens associated with sexually transmitted diseases and urinary tract infections. By comprehending the mechanisms behind inflammatory signatures, contemporary research is aimed at reducing PCa mortality rates and enhancing the effectiveness of treatment strategies.
