Respiratory bronchiolitis with interstitial lung disease (RBILD), smoking-related interstitial fibrosis (SRIF), and desquamative interstitial pneumonia (DIP) present a confusing pathologic problem, because RBILD and smoker’s respiratory bronchiolitis (RB) are morphologically identical but differ in imaging findings, with clinical/radiologic evidence of an ILD transforming RB into RBILD. RBILD is believed to evolve in some instances into DIP; DIP is frequently impairing and may progress to end stage fibrosis. SRIF is a morphologically dramatic but distinctly localized form of fibrosis that does not progress to functionally impairing disease.
