This chapter covers the concept of acute lung injury, current definitions of acute respiratory distress syndrome (ARDS), and its pathologic correlate, diffuse alveolar damage (DAD). Acute interstitial pneumonia (AIP) is discussed as a form of ARDS developing in previous healthy patient and for which the etiology of the ARDS is not apparent. The clinical, radiologic, and pathologic features of AIP/ARDS/DAD are reviewed, along with a pathologic differential and information on treatment and prognosis.
