There are several diseases characterized by granulomas and various types of interstitial lymphocytic inflammation that are either uncommon (common variable immunodeficiency (CVID), selective IgA deficiency) or that infrequently produce pulmonary symptoms and hence are infrequently biopsied (primary biliary cholangitis (PBC), Crohn’s disease, ulcerative colitis). Morphologically, these entities often have overlapping features, and all of them can show lymphoid hyperplasia. History is crucial for correct assessment.
