Cardiac MRI

The left ventricle and ascending aorta undergo significant alterations in patients with bicuspid aortic valve (BAV), the most common congenital heart defect with a prevalence of 0.4–2.25%. Valve dysfunction, including aortic stenosis and aortic regurgitation, frequently leads to progressive ventricular remodeling and complications such as ascending aortic dilation and aortic dissection. Multimodality imaging, particularly cardiac magnetic resonance (cardiac MRI), has proven crucial in providing an accurate evaluation of valve anatomy, ventricular function, and myocardial tissue characterization. This review explores the role of cardiac MRI in managing BAV patients, highlighting its utility in left ventricular morpho-functional assessment, quantitative evaluation of valve dysfunction, and myocardial tissue characterization. Additionally, the limitations and future perspectives of cardiac MRI, including the application of 4D Flow sequences for hemodynamic analysis, are discussed.
