Concussion Treatment

The initial cornerstone of management after a concussion is removal from competition and prevention of reinjury, followed by a brief period of relative physical and cognitive rest. Education around recovery expectations is critical for patients and families. Structured exercise below the level of significant symptom exacerbation has been shown to be beneficial for recovery. Prolonged periods away from school/work should be avoided due to the detrimental effects caused by social isolation. Post-concussion headaches are common and usually managed with acetaminophen or nonsteroidal anti-inflammatory drugs (NSAIDs). Physical therapy and OMT have been shown to be effective for dizziness and cervicogenic headaches. No current medications are approved for improving cognitive symptoms after concussion, although amantadine and stimulant medications have been used with some success. Sleep is critical for recovery and education on sleep hygiene is necessary. Melatonin is reasonable to consider if needed. Data on supplement efficacy is sparse, but a multivitamin with C, D, and E, along with omega-3 is reasonable to recommend.
