Aortic aneurysms (AA) affect 5% of men over 65 years old and 1% of women. They tend to grow as time goes by and may eventually rupture (mortality rates up to 80%). The treatment of AAs is divided into open and endovascular repairs. Surgical repair proved to be an effective way to reduce the mortality associated with rupture. Minimally invasive endovascular aneurysm repair (EVAR) has gained worldwide acceptance. The percentage of patients who could benefit from EVAR is increasing with the improvements and maturity of its technology. Patients who are fragile and old may have access to treatment with a success rate overpassing 95%. Should side branches be involved, alternative techniques and innovative devices can make more than 90% of people amenable to EVAR. The basic design of all endografts is somewhat similar: a tubular fabric graft supported by stents. Devices with lower profiles and/or modified designs including fenestrations have the potential to treat more challenging anatomies. The durability of the materials, suitable radial support, and better anchorage guarantee the long-term life of the device. Improvements in understanding the characteristics of endovascular technologies are helping select the best treatment for people with AAs.
