Deep Neck Lift

This chapter outlines the authors’ preferred approach for comprehensive contouring of the neck. Often, traditional liposuction of the neck offers a limited treatment of the subcutaneous plane of the neck without addressing platysma laxity/malposition or subplatysmal fat. Also, liposuction of the subcutaneous fat can lead to fibrosis of the dermis to the platysma with resulting contour deformities. The main advantages of the deep neck lift include optimal neck contour in patients with subplatysmal fullness as well as correction of platysmal banding while avoiding over-resection of subcutaneous fat. In fact, in the authors’ experience, it is surprising how little of the subcutaneous fat needs to be removed to achieve a satisfactory neck contour. This chapter outlines technical details of the operation, anatomic considerations, and postoperative management.
