Complications 2: Dyspigmentation

Dyspigmentation after resurfacing procedures can be a challenging condition to resolve. This complication can present most commonly as hyperpigmentation but may also appear as pseudo-hypopigmentation or rarely as true hypopigmentation following injury or inflammation of the skin. Rapid recognition of this entity with expedited sequential treatment is essential for a favorable outcome. At-home skin preconditioning performed by the patient is perhaps one of the most important steps in treating post-inflammatory hyperpigmentation (PIH). Simple but effective regimens are comprised of a retinoid, hydroquinone, or alternate lightening agents and physical sunscreen. When PIH is more persistent, superficial chemical peels can be safely employed in all skin types to help lift unwanted pigmentation. For recalcitrant cases, laser or light-based therapies can offer some improvement. In cases of pseudo-hypopigmentation, the resurfaced skin appears lighter in comparison to nearby non-treated skin. Superficial peels with topical lighteners can blur the lines of demarcation and minimize contrast. For true hypopigmentation, resurfacing lasers, phototherapy, or focal light-based therapies and topical agents can encourage repigmentation.
