Extra-articular soft tissue pathology following total hip arthroplasty (THA) can cause significant pain and disability despite an otherwise well-executed primary surgery. Once component malposition, instability, infection, and other component-related causes for failure have been diagnostically eliminated as etiologies of persistent grievance following THA, it is crucial to evaluate the integrity and function of extra-articular entities such as adjacent muscular, tendinous, and ligamentous structures. This chapter explores diagnosis, treatment, and management strategies for musculotendinous soft tissue pathologies arising from, or exacerbated by, total hip arthroplasty. Specifically, this chapter expands upon the workup of lateral peritrochanteric hip pathology and hip flexor pathology following THA.
