In select cases, an allograft prosthetic composite (APC) is a viable option for revision total hip arthroplasty. Indications include bone loss in the proximal femur and/or trochanter, lack of abductor attachment, reconstruction due to prior infection or osteolysis, or significant B3 periprosthetic fractures. It consists of a femoral prosthesis surrounded by a proximal femoral allograft, which restores native bony anatomy and providing an attachment site for the abductors. Variations in technique have been described but generally include a femoral osteotomy and cerclage wires, plate, and/or allograft fixation at the graft–host junction. Once the junction is healed, the implant is load sharing and provides overall good long-term stability and functional outcomes. Complications include nonunion, dislocation, fracture, loosening, infection, hematoma, and greater trochanter migration. Overall, survivorship can be as good as 80% at 10–15 years.
