Two-stage exchange arthroplasty with a static or articulating spacer in the interim period remains the preferred treatment for chronic periprosthetic hip joint infection. Static spacers, also known as non-articulating spacers, are designed to remain fixed in place and do not allow joint movement. Insertion of a static versus articulating spacer often depends on surgeon preference and patient factors such as extensive bone loss or severe soft tissue damage. Static spacers deliver high concentrations of antibiotics to the infected site but do not preserve hip joint function in the interim. Advantages include a simpler reconstruction, lower costs, and reduced stress on the surrounding bone. This chapter will review the indications, pros and cons, technique, complications, and outcomes of static spacer insertion in the setting of two-stage exchange arthroplasty for treatment of periprosthetic hip joint infection.
