The management of acetabular bone loss has continued to evolve over time. Understanding the pattern, severity, and location of bone loss is imperative to achieving a stable, durable, and painless reconstruction. Customization with 3D printing and highly porous metals has changed the role in which cages and rings are used. Cages and rings now have a limited role as complimentary devices for the highly porous hemispherical cup and are used in isolation in rare cases when a highly porous cup is not able to achieve a press fit or stable rim fixation, even when combined with an augment. Additionally, they are an affordable alternative to a custom triflange, particularly among elderly patients that cannot tolerate the long wait required to manufacture these custom implants. These devices in isolation still can be used in a pinch, and it is important to understand their unique role when dealing with complex acetabular reconstructions.
