In revision total hip arthroplasty, the goals of revision acetabular reconstruction include establishing mechanical strength of fixation, a high degree of durability, and the eradication of instability and infection. Throughout history, various techniques have emerged for revision of acetabular components including cemented cups and cages, first-generation uncemented cups, porous cups, augments, cup/cages, custom implants, and technology and porous coatings. The uncemented hemispherical shell has risen as the workhorse for revision acetabular reconstruction. These shells can be used alone or in combination with bone grafting, augments, cages, or with other constructs cemented inside. This chapter will focus on the role of hemispherical cups in the setting of revision total hip arthroplasty.
