The anterior-based muscle sparing (ABMS) approach is a modified version of the classic anterolateral approach. The ABMS, or alternatively called anterolateral minimally invasive (ALMI) approach, involves sparing of the abductor muscles using an intra-muscular approach. While the posterior and direct anterior approaches are favored for primary hip arthroplasty, it may be appealing to use an alternative approach for revision surgery to avoid scar tissue and other debris. The ABMS approach, compared with other approaches, can be accomplished with similar operative time and outcomes compared with other approaches. This approach can also have a lower risk of dislocation and earlier recovery due to the fact that muscles are left intact. Specifically in the context of revision arthroplasty, the enhanced exposure of both the acetabular and femoral anatomy can also be advantageous. Countering these benefits, surgeons must have familiarity with this approach to avoid injury to surrounding structures and to ensure a proper envelope for the prosthesis. Also, the ABMS approach is not suitable for cases where access to the posterior column is required.
