The direct lateral approach in total hip arthroplasty (THA) involves dissecting through the lateral hip muscles, allowing precise implant placement and stability. This chapter outlines the surgical method for primary and revision THA using this approach, covering steps from incision to closure. Advantages include lower dislocation rates and decreased risks to nerves and vessels. However, it is essential to handle abductor muscles carefully to prevent complications. Overall, the direct lateral approach provides effective exposure for both acetabulum and femur in primary and revision THA. By ensuring meticulous closure of the anterior tissue cuff and avoiding nerve damage, this approach can lead to excellent postoperative stability and prompt functional recovery.
