The direct anterior approach (DAA) for total hip arthroplasty (THA) is a minimally invasive and soft tissue-sparing approach that exploits an inter-nervous plane to the hip. The DAA has reportedly been shown to improve early postoperative functional recovery and also have a reduced dislocation risk. Over the past couple of decades, this approach has gained in popularity amongst surgeons, due to these advantages, in the setting of a growing shift towards outpatient total joint arthroplasty and desire to decrease hospital length of stay. While there is an associated learning curve with this technique, more widespread adoption of the DAA will steadily improve this amongst future surgeons as more and more trainees gain exposure. It is important to be aware of the associated complications involved with this approach, namely femoral sided fracture. Through careful surgical technique and femoral exposure, surgeons can optimize their outcomes using this efficient approach while minimizing the risk of complications.
