Revision total hip arthroplasty requires thorough evaluation and classification of the acetabular component and possible associated bone loss to guide treatment strategies. It is important to obtain an adequate history, physical exam, radiographs, laboratory values, existing implant information, and advanced imaging to understand the modes for failure. If present, acetabular bone loss must then be carefully evaluated and classified to drive management decisions. Although limited, there are many classifications that exist with their own strengths and limitations. Once this comprehensive evaluation has been completed, surgical approach and implant choice can then be planned, taking into account the patient’s overall health, mode of failure, and adequate remaining bone stock.
