This is a case with metastatic clear cell renal cell carcinoma, who developed PMR-like syndrome after 3 cycles of combination ICI therapy of ipilimumab and nivolumab. Clinically, he had the classic presentation of neck and proximal muscle pain and stiffness, although his inflammation markers were only mildly elevated. He responded well to 20 mg of prednisone, however he failed several attempts at tapering prednisone off despite adding sarilumab, but finally able to achieve clinical remission with combination of sarilumab and low dose maintenance prednisone at 7.5 mg daily.
